Permits and Forms

DOG LICENSE APPLICATION:

Required by Wisconsin Statue 174.07(1)(a) for any dog more than 5 months of age on January 1st or that will be 5 months of age within the license year. Landlords are required to advise renters!

A temporary Picnic License is required anytime BEER will be sold at any event, including ball games, church picnics or special Holiday events.

Town of Herman Town Hall Rental Fees:

Town of Herman residents or landowners: $60 per day plus $50 deposit.

Non-resident: $100 per day plus $50 deposit.

Leopolis Park Shelter Rental Fees:

Town of Herman residents, landowners and Leopolis associated residents: $50 per day plus $50 deposit.
